PCPS Offering New Health Services Program for Students

Polk County Public Schools is launching a new health services program, and we want to make sure your family has access all year long. The school district is partnering with Hazel Health, a telehealth provider for K-12 schools, to expand services for students. With Hazel, students can immediately speak with a licensed medical professional or mental health clinician right from home or in school. This service is now available for all families from home and in schools. When a student is in school, the school nurse or designated staff uses an iPad to help them talk directly with a medical professional over live video. During Hazel’s virtual visits, students can receive treatment for common ailments such as stomach aches and headaches, as well as mental health counseling for anxiety, depression and other issues. Hazel services are provided at no charge for all students at this time. PCPS is hosting a Consent for Care drive to ensure all families know about this new telehealth service available for students. Parents and guardians must give consent if they want their children to receive health care services from Hazel Health. To learn more about Hazel Health and submit a Consent for Care form to opt-in to this new service, visit my.hazel.co/polkschoolsfl On-demand physical health services include: • Medical treatment: for stomach aches, headaches, sprained ankles, asthma concerns, and everything in between • Medication: prescriptions written or over-the-counter provided at school when needed • Coordination: with family physician and return-to-school communications Mental health services include: • Scheduled short-term therapy visits: no waitlist for scheduled therapy sessions for coping with everything from anxiety, depression and grief, to peer and family relationships and bullying • Care management to local resources: referrals to local providers for continuity of care • Coordination: communication with families, local health providers and school staff
